BACKGROUND CHECK

All adults working with youth, 17 and under, must complete our Volunteer Application and our ministry online background check PRIOR to working with youth. This includes staff, volunteers, coaches, etc.

Using your email address, we will send you a link to complete the background check. Once you receive it, the link is only active for 48 hours. Therefore, once the process is initiated time is of the essence.

Individuals with “Sex Offender” status will not be permitted to volunteer in any facility during the time youth are present. Other recent violent felonies may also preclude a volunteer from serving the youth as well.

Our Boxing Program Volunteer requirements will be governed by the AAU Boxing Association and possibly other Boxing protocols subject to the Lord’s Gym Ministries’ discretion.

I understand FOCAS reserves the right to conduct criminal background and reference checks on all volunteers. I hereby give permission for FOCAS to obtain information relating to my criminal history record. The criminal history record, as received from the reporting agency, may include arrests and convictions. I understand that this information will be used, in part, to determine my eligibility for a volunteer position with FOCAS. I also understand that FOCAS may continually monitor active volunteers’ criminal records by performing periodic criminal record and background checks. I understand that I will have an opportunity to review the criminal history and a procedure is available for clarification, if I dispute the record as received.

My electronic signature below indicates I have read and acknowledge this statement this statement and offer my approval for FOCAS to conduct a criminal records check if required for this volunteer activity. I understand that any and all continued volunteer service with FOCAS is dependent upon the results of my criminal history record, reference checks and any other documents required that need to be verified.
